Case Manager

Report to Associate Director of Client Services
Duties and Responsibilities include:
- Provide direct case management and advocacy services to a case load up to thirty-five (35) clients living with HIV/AIDS.
- Leading and be responsible for service delivery, required documentation and recruitment goal of a four-people case management team.
- Meeting contractual requirements.
- Identify and recruit Asian and Pacific Islanders who are living with HIV/AIDS to facilitate early access to treatment and social services.
- Working as a part of the unit at the quality assurance and continue quality improvement (CQI) tasks.
- Maintain client-related records and other required documentation according to the protocols and standards of APICHA’s Client Services Department.
- Represent the agency at various venues to promote agency services.
- Other activities as assigned.
Qualifications:
- Bachelor’s degree in social science, psychology, social work, health or equivalent experience required. Advanced degree in above mentioned field strongly preferred.
- At least one year of full-time work experience preferred.
- Experience in working with at least one of the following communities is required: people living with HIV/AIDS, Asians and Pacific Islanders, immigrants, gays, lesbians, bisexual and transgendered persons, and families with children.
- Fluency in one or more Asian language (Mandarin or Cantonese preferred).
- Excellent written and verbal communication skills in English.
- Independent and good team player.
- Excellent people skills.
- Good attention to details.
- Ability to follow through instructions.
- Ability to handle multiple tasks and stressful environment.
- Ability to work well within the organizational structure.
- Ability to work flexible hours including some evenings and weekends.
- Ability to use common office software. (Word, Excel, Power Point). Familiarity with URS/AIRS preferred.
