Clinical Social Worker

Reports to: Director of Programs

Job Duties and Responsibilities:

  • Conduct psychosocial assessment, diagnosis, create treatment plan, and implementation of treatment plan to patients in Primary Care setting
  • Complete charting in EMR and required paperwork in a timely manner
  • Assist clients with any other needs, including entitlement assessment and advocacy in the field required.
  • Collaborates with multidisciplinary team; attend team/clinic/Agency meetings
  • Participate in the development/review of clinic mental health policies/procedures
  • Practice within the scope of license, training and credentialing, and in accordance with current standards of care and quality criteria adopted by the clinic and the NYSDOH
  • Actively participate in quality improvement program and program evaluation
  • Participate in patient recruitment activities
  • Maintain confidentiality of patient care and agency operations at all times
  • Work with the support and administrative staff in a positive, constructive manner
  • Produce reports on activities in a timely manner
  • Perform other duties as required or assigned

Qualifications:

  • License to practice mental health services in NYS is required. LCSW with R privilege is not required but preferred.
  • Excellent written and verbal communication skills in English required.
  • Knowledgeable of MSM/LGBTQI issues required.
  • Ability to work with people from diverse backgrounds in multi-cultural setting required.
  • Ability to work with Limited English Proficient populations and interpretation support required
  • Familiarity with immigrants especially Asian & Pacific Islanders required.
  • Proficiency in common office software (Word, Excel), and ability to enter data accurately required.
  • Ability to work flexible hours including some evening(s) and possibly some weekend required.
  • Ability to take public transportation required.
  • Being pleasant and personable over the phone as well as in person required.
  • Ability to work well with staff and patients
  • Ability to work independently and as part of a team within a hierarchical organizational structure.
  • Knowledge of AIRS and eClinicalWorks software is preferred.
  • Fluency in one or more Asian language is preferred
