Clinical Social Worker
Reports to: Director of Programs
Job Duties and Responsibilities:
- Conduct psychosocial assessment, diagnosis, create treatment plan, and implementation of treatment plan to patients in Primary Care setting
- Complete charting in EMR and required paperwork in a timely manner
- Assist clients with any other needs, including entitlement assessment and advocacy in the field required.
- Collaborates with multidisciplinary team; attend team/clinic/Agency meetings
- Participate in the development/review of clinic mental health policies/procedures
- Practice within the scope of license, training and credentialing, and in accordance with current standards of care and quality criteria adopted by the clinic and the NYSDOH
- Actively participate in quality improvement program and program evaluation
- Participate in patient recruitment activities
- Maintain confidentiality of patient care and agency operations at all times
- Work with the support and administrative staff in a positive, constructive manner
- Produce reports on activities in a timely manner
- Perform other duties as required or assigned
Qualifications:
- License to practice mental health services in NYS is required. LCSW with R privilege is not required but preferred.
- Excellent written and verbal communication skills in English required.
- Knowledgeable of MSM/LGBTQI issues required.
- Ability to work with people from diverse backgrounds in multi-cultural setting required.
- Ability to work with Limited English Proficient populations and interpretation support required
- Familiarity with immigrants especially Asian & Pacific Islanders required.
- Proficiency in common office software (Word, Excel), and ability to enter data accurately required.
- Ability to work flexible hours including some evening(s) and possibly some weekend required.
- Ability to take public transportation required.
- Being pleasant and personable over the phone as well as in person required.
- Ability to work well with staff and patients
- Ability to work independently and as part of a team within a hierarchical organizational structure.
- Knowledge of AIRS and eClinicalWorks software is preferred.
- Fluency in one or more Asian language is preferred
