Care Coordinator
Reports to Associate Director of Client Services.
Duties and Responsibilities Include:
- Provide supervision to patient navigator staff by chart review, face to face case discussion and performance reviews.
- Responsible for implementation of service plan, supported by Patient Navigators.
- Coordinates and oversees the implementation of the comprehensive plan.
- Provides clinic-based health education in conjunction with regularly scheduled primary care visits.
- Performs the initial entitlements assessment and collates the comprehensive plan.
- Facilitates interdisciplinary conversation and planning.
- Provides backup to Patient Navigators as needed.
- Collate data reports (e.g., appointment dispositions, laboratory results, etc.) produced by clinic information systems.
- Collaborates with the clinical team to manage care coordination resources and target the neediest PLWHA for service.
- Maintain client-related records and other required documentation according to the protocols and standards of APICHA’s Client Services Department.
- Working as a part of the unit at the quality assurance and continue quality improvement (CQI) tasks.
- Represent the agency at various venues to promote agency services.
- Meeting contractual requirements.
- Other activities as assigned.
Qualifications:
- BA/BS or LPN and two year of case management experience required. Advanced degree in related field may substitute 2 years of case management experience.
- At yeast one year of full-time work experience preferred.
- Experience in working with target population preferred.
Fluency in one or more Asian language preferred.
- Excellent written and verbal communication skills in English required.
- Proactive problem solver who works well in multi-disciplinary setting.
- Good attention to details and meeting tight deadlines.
- Ability to communicate well with medical providers and support staff.
- Ability to work well with diverse population.
- Ability to handle multiple tasks and stressful environment.
- Ability to work well within the organizational structure.
- Ability to work flexible hours including some evenings and weekends.
- Ability to use common office software. (Word, Excel, Power Point).
- Familiarity with AIRS and eCW preferred.
