Getting primary medical care at APICHA Community Health Center is fast and easy. Call us at (212) 334-6029 to schedule an appointment.
We are accepting new patients and you can get an appointment fast. Patients receive care regardless of race, ethnicity, sexual orientation, age, immigration status, or ability to pay.
Your First Visit
Your first appointment at the clinic will be a short visit. You will fill out our easy to understand forms (download form) and have your blood drawn by a Medical Assistant. Please make sure to fast for 8-10 hours prior to your appointment.
Things to bring with you at your first visit:
- Photo ID – any official picture identification card, such as a passport, driver’s license or work ID
- Proof of address – utility bill, recent mail.
- Insurance information.
- if you don’t have insurance, bring proof of income such as pay stub, bank statement or letter from your employer. See FAQ below if you do not have proof of income.
Your Second Visit
At your second appointment you will meet your Primary Care Provider (PCP) who will ask you about your health history and explain the results of your blood tests.
At this appointment, you and your PCP will create a care plan that works for you.

Wednesdays and Thursdays 9:15 AM – 7:00 PM

What insurances do you take?
We take most insurances (click here for the most updated insurance list). You cannot find your insurance on the list? Call us for more information.
Can I come to your health center if I don’t have insurance?
YES. You may qualify for our low cost/sliding scale.
Can I be seen without proof of income?
If you do not have proof of income, you will be asked to sign a Financial Self-Declaration Form (download form) that certifies your income for financial assistance. This helps our staff determine your eligibility for a discounted fee schedule.
