Case Management for HIV
As an enrolled client you will get a case manager, who can assist you with social services needs and entitlements if you qualify, so that you can focus on improving your health. We create a safe and comfortable space for you to discuss your needs. Our goal is to provide assistance in treatment adherence for those who are living with or are affected by HIV/AIDS. Our services are confidential and FREE of charge regardless of your gender, sexual orientation, immigration status, nationality, religion, or creed. Our Transgender Health Clinic provides individualized primary medical care to people from all backgrounds in an affirming and safe environment. These services include:
PrEP or Pre-Exposure Prophylaxis is a medication approved by the U.S. Food and Drug Administration (FDA) for daily use that can help people stay HIV-negative. PrEP, or Pre-exposure prophylaxis, helps reduce HIV infection rates. When taken daily as directed, PrEP can reduce the risk of HIV infection by more than 90 percent.
Our program provides screening to individuals to see if PrEP is right for them. Those who qualify are provided with a prescription to start taking a daily medication called Truvada, a two-drug combination in one pill.

If you and your Apicha CHC medical provider believe that you would benefit from behavioral health services, you must first undergo a psychosocial assessment, in order to determine your eligibility to receive limited short-term counseling or a referral for ongoing psychotherapy.
Apicha Community Health Center’s social workers can provide you with short-term counseling or a referral to another provider depending on your individual needs. Our psychiatrist is available for an initial evaluation and medication management.
Please note: With all Apicha Community Health Center patients, a referral from your primary care provider for a psychiatric evaluation or medication management is required.

Apicha Community Health Center’s Behavioral Health Services are not meant for patients who have been diagnosed with severe or chronic mental or behavioral health issues. We will provide referrals to patients who require long-term psychotherapy.
Please note: With all patients, a referral from your primary care provider for a psychiatric evaluation or medication management is required.

Apicha Community Health Center has resumed in-person HIV and STI Testing, in addition to participating in New York City Department of Health and Mental Hygiene's Community Home Test Giveaway (CHGT). In partnership with Hunger Solutions, NYC, Nutrition Outreach and Education Project (NOEP) seeks to increase participation in Supplemental Nutrition Assistance Program (SNAP), to reduce hunger in New York City.
NOEP does this by providing individuals and households with confidential and accurate pre-screenings and application assistance, and partnering with community sites to participate in School Breakfast Program and the Summer Food Service Program.

Apicha CHC's Enroll Manhattan Project seeks to help New Yorkers obtain care, through understanding health insurance, how it can best work for them, and helping those who are uninsured and underinsured enroll in health insurance through the New York State of Health Marketplace.

Project Connect’s youth initiative, GAYME (GLBTQI API Youth Mentorship for Empowerment), connects LGBTQI API youth with other youth and adults to share their experiences. The group format is utilized to provide a safe and supportive environment.

A Healthier Me! is Project Connect's skills-building workshop series that invites expert speakers from the larger communities to lead workshops and lectures. Many of our speakers work specifically with LGBTQI and/or API individuals in their profession and many identify as LGBTQI API. All speakers are deeply committed to serving the LGBTQI API community and have tailored their workshops to address issues relevant to the LGBTQI API population.

Apicha Community Health Center's Community Mobilization Project teams up with other organizations in our community to create special events tailored for New York City’s LGBT communities, people living with HIV/AIDS, and people of color.
At these events we will enjoy the work created by our communities and learn about ways to stay like Viral Load Suppression and PrEP.
It is our hope that these events will provide a space for these communities to come together, celebrate our work, struggles, and achievements, and, most importantly, inspire one another.
These events are sponsored through a grant from the New York State Department of Health’s AIDS Institute
